a new user ID and pick your own password

Enter
your first and last name, click continue

Click
edit profile, enter your information, and click “save profile”

Click “Start
Shopping …” button - choose the form that work best
for you

Follow
the prompts

Enter
an order…if you don’t want it, click the “cancel
order” link
in the lower right of every screen.

If you
want the order, click the final approval button after the credit
card information
-
Print
out the confirmation screen that follows to record your order date
and ID #.
